Please read and agree to all three disclosures. Your application cannot be processed without your consent.

In connection with your application for employment, AGY Logistics (the "Company") will obtain a consumer report and/or investigative consumer report (commonly referred to as a "background check") for employment purposes. These reports may include, but are not limited to, the following types of information: criminal history, driving records (MVR), employment history verification, education verification, and other public-record information. The reports will be obtained from a consumer reporting agency (CRA) in compliance with the Fair Credit Reporting Act (FCRA), 15 U.S.C. § 1681 et seq. The CRA(s) that may be used will be provided to you upon request. Information gathered will be used solely for employment purposes. You have the right to request disclosure of the nature and scope of any investigative consumer report.

As a condition of employment and continued employment, all safety-sensitive employees (including CDL-A drivers) are subject to the Company's DOT-mandated drug and alcohol testing program in accordance with 49 CFR Part 382 and Part 40. This includes: pre-employment drug testing; random drug and alcohol testing; reasonable-suspicion testing; post-accident testing; return-to-duty testing; and follow-up testing. Refusal to submit to testing, tampering with a test, or a verified positive result will result in immediate removal from safety-sensitive duties and may result in termination of employment. By applying, you acknowledge you have read and understand this policy.

You agree to use electronic signatures for this application and any associated documents. Your typed name constitutes your legally binding electronic signature under the Electronic Signatures in Global and National Commerce Act (E-SIGN Act) and applicable state law. Electronic records provided to you may be in PDF or HTML format. You have the right to receive a paper copy of any record upon request. This consent may be withdrawn at any time by contacting us in writing; however, withdrawal will not affect the legality of actions already taken in reliance on your consent.
